ONLINE HMO PRE-APPROVAL PROCESS:

• Please complete the forms to process your HMO request one day before your visit.

• Kindly ensure that you complete the requested tests within 3 days of receiving approval.

• Please allow us at least 24 hours to process your request once it is submitted.

• Please note that we still need to contact your health insurer for approval. Wait for our email or text confirmation before proceeding to the HMO Office.

Please be aware that Online Approvals are processed from 8:00 AM to 3:30 PM (Monday to Sunday), requests that will be sent to us beyond the given time frame will be accommodated the following day.
